First edition. The personal copy of Admiral Charles S. Butler, who served in the United States Navy Medical Corps, with a presentation note to him from a David Ferguson mounted on front flyleaf of second volume. Bindings professionally restored some time ago, probably by Butler based on Ferguson's comment in the note: 'It is a shame the way Vol. II is breaking up from the viscissitudes of handling by Navy Rackers.' Another more general presentation note is written by Ferguson on the front endpaper of the first volume. The second volume's flyleaf is loose, probably left so intentionally by the binder so the note could be laid in. The presenter is almost certainly Capt. David W.
